This Kind of Tired Doesn't Go Away With a Good Night's Sleep

conversation, a month that asked too much of you. And then there’s the kind that just stays. The kind where you wake up already exhausted, where small things set you off in ways you’re not proud of, where you’ve been telling yourself you’ll slow down once things settle, but things never quite settle.

That second kind isn’t a productivity problem. It’s not solved by a weekend away or a better morning routine. When stress becomes the background noise of every single day, it starts doing real damage to your sleep, your health, your relationships, your ability to think clearly. That’s when it becomes something worth treating.

At Bio Psychiatry Therapeutic Services, LLC, we work with people who’ve hit that point. Our provider, Vida Robb, PMHNP-BC, takes a holistic look at what’s driving the stress, what it’s doing to the body and mind, and what kind of support will actually make a dent. All of this happens through telepsychiatry, so you can get real care without adding another obligation to an already full plate.

How Do You Know When Stress Has Become a Problem?

Most people don’t walk in saying ‘I think I have chronic stress.’ They come in because they can’t sleep. Or they keep getting sick. Or their partner told them they’re not the same person they used to be. Stress tends to announce itself through other things and by the time those things are obvious enough to act on, it’s usually been building for a while.

Some of what we look for during an evaluation:

Your body is keeping score

Headaches that show up most days. A jaw you’re clenching without realizing it. A stomach that reacts to things it didn’t use to. Fatigue that doesn’t improve no matter how much you rest. Chronic stress has physical effects that are well-documented the body runs a low-grade stress response for so long that it starts to show up in the body in very concrete ways.

Your mind won’t let you switch off

You’re lying in bed at midnight running through conversations from three days ago. You sit down to do one thing and end up doing nothing because the mental load is so heavy that starting feels impossible. Your concentration is patchy. You forget things you shouldn’t forget. The mental noise just doesn’t stop.

Your emotions are harder to manage than usual

Shorter fuse. Less patience. A flatness where there used to be more feeling. Or the opposite emotions that feel closer to the surface and harder to contain. People under chronic stress often describe feeling like they’re running on empty emotionally, like there’s just nothing left to give by the time they get home.

You’ve stopped doing things that used to help

Exercise has dropped off. You’re not seeing people as much. You’ve been leaning on a glass of wine or a few extra hours of scrolling to decompress. These aren’t moral failures, they’re patterns that tend to emerge when the nervous system is overwhelmed and just looking for relief wherever it can find it. But they also tend to make the stress worse over time.

It’s starting to look like something else

Stress that goes unaddressed often tips into anxiety or depression, and sometimes it’s hard to tell which came first. Part of what we do at the assessment stage is sort out that relationship. Because the treatment for stress that’s caused anxiety looks different from anxiety that’s masking as stress, and getting that distinction right matters.

What Treatment Actually Looks Like Here

We don’t hand out generic advice. What we do is look carefully at what’s going on for a specific person and put together something that fits their actual life.

Starting with a thorough evaluation

The first step is always understanding the full picture. That means a real conversation about what’s happening now, what’s happened before, what your health history looks like, whether there’s anything else going on clinically. We’re not looking to rush to a solution. We’re looking to actually understand what we’re dealing with.

Therapy that targets the patterns keeping you stuck

A lot of chronic stress is held in place by the way we think about what’s happening to us, the tendency to assume the worst, to take on more than is ours to carry, to confuse urgency with importance. Cognitive Behavioral Therapy helps people start to see those patterns clearly, question them, and gradually replace them with responses that don’t leave them depleted. It’s practical work, not abstract, and it gives people tools they actually use.

We also draw on mindfulness-based approaches where they fit not as a cure-all, but as a way of helping people build some distance from the constant noise in their heads.

Medication, when it makes sense

Not everyone needs medication for stress. But when chronic stress has triggered a depressive episode or an anxiety disorder, or when someone is so overwhelmed that therapy can’t get traction yet, medication can create enough stability for real progress to happen. Our provider approaches this carefully prescribing when it’s genuinely indicated, monitoring closely, and adjusting based on how the person is actually doing.

Practical changes that hold up in real life

Sleep, boundaries, how someone spends the margins of their day these things matter and we talk about them. Not in a ‘have you tried journaling?’ kind of way, but with real specificity. What would make next month feel more manageable than last month? What one thing, if it changed, would take the most off your plate? That’s the kind of conversation we have.

We See Clients at Every Stage of Life

Stress doesn’t look the same at eight as it does at forty-five. A child dealing with school pressure and a parent dealing with financial strain and a senior navigating health changes and loss they’re all carrying weight, but it looks and feels different for each of them. We work with children, teens, adults, and seniors, and we adjust how we work based on who’s in the room.

All sessions are held via telepsychiatry, which means wherever you are in Maryland or DC, you can access the same quality of care without the commute.

You'll Work with Someone Who Actually Knows Your Story

One of the things clients tell us matters most is continuity. Not starting over with someone new every few visits. Not having to re-explain your history every time. At Bio Psychiatry Therapeutic Services, LLC, you’ll work with a provider who knows where you started and tracks how things are changing and who adjusts the plan accordingly, not just at the beginning but all the way through.

We accept most major insurance plans across Maryland and DC, and we also see self-pay clients. If you’re not sure whether you’re covered, just reach out before booking, we’re happy to help you sort that out.
